There is a $10 fee for form completion when the request is not associated with an appointment. Also, the patient must have been seen by an Apple Ridge doctor/nurse practitioner within the past 6 months. All FMLA forms must be accompanied by an appointment and are not covered by the form fee.

New Patient

Apple Ridge Family Medicine is not accepting new patients at this time. Please check back with us in January 2024.

Opioid/Alcohol Abuse Treatment
